Emergency Cards
A little lifesaver of a card.
Minerva allows you to create your own Emergency Cards. Select one or all of the Emergency Cards: Emergency Identification Card, Medical Alerts Card, or an Organ Donor Card.
Each of the Emergency Cards can be printed and kept in your wallet right next to your driver’s license and Minerva Flash Drive, making them available for immediate access by
emergency personnel. And any time you update your personal health information in Minerva, changes will be immediately available so you can print new Emergency Cards to keep vital
emergency information current and with you at all times.
Here are the details of the Minerva Emergency Cards:
The Emergency Identification Card provides a summary of selected health information that was entered about your Personal Profile, Current Residence, and Emergency Contacts (up to three contacts are shown on the card).
The Medical Alerts Card identifies if a Medical Alert has been flagged for any entry in the following Minerva health topics:
- Health Conditions - Allergies, Diagnoses, Devices and Aids, Pregnancy
- Medications - Prescription Drugs, Non-Prescription Products, Vaccinations
- Treatments - Surgical and Non-Surgical Procedures
The Organ/Tissue Donor Card shows the donor wishes and instructions that were entered in the following Minerva health topic:
- Future Care Decisions - Organ/Tissue Donations
The Minerva Flash Drive Instructions card provides information on how to view the Emergency Health Records and Personal Health Records that have been exported to the Minerva Flash Drive.
